The Mishnah Berurah, Magen Avraham, Taz, and Aruch HaShulchan all rule that the apple requires its own bracha of borei pri ha-etz even during a bread meal, because it is not eaten on account of the meal (she-lo me-chamat ha-seudah). The apple is the primary item and the honey is secondary.
