For a walled city, is the Shabbat boundary measured from the wall or from the inhabited structures, depending on whether habitation preceded or followed enclosure?

Synopsis

If a city was first inhabited and then enclosed by a wall, the boundary is measured from the wall. If the wall was built first and the city was inhabited afterward, the boundary is measured from the inhabited structures, not from the (outer) wall.
