Should Mizmor L'Todah (Psalm 100) be recited in a melody, and on which days is it omitted?

Synopsis

All authorities agree that Mizmor L'Todah should be recited with a melody. The Rema (Ashkenazi custom) omits it on Shabbat, Yom Tov, all of Pesach, erev Pesach, and erev Yom Kippur because the Thanksgiving offering was not brought on those days. There is a dispute about Rosh Hashanah, Yom Kippur, and Tisha B'Av.
