The Rema records the Ashkenazi customs of reciting Yizkor, blessing the congregation workers, Yekum Purkan, and Av HaRachamim after the Torah reading. Av HaRachamim and Yizkor are omitted on Shabbatot when Tachanun is not said, with exceptions for the Sefirah period and other cases.
