Should the one who breaks bread (botze'a) intend to discharge all participants' obligation of lechem mishneh, and should they recite 'birshut'?

The Mishnah Berurah recommends that the botze'a intend to discharge all participants with his HaMotzi blessing, and that the participants intend to be discharged. He also states the botze'a should say 'birshut rabbotai' (with permission of my masters) even if he is the head of the household, as a matter of humility. The Aruch HaShulchan states this is the ideal practice so that women at the table also fulfill lechem mishneh through the botze'a.
