What constitutes hatmanah (insulation) as opposed to shehiyah (leaving food on a heat source)?

Synopsis

Hatmanah requires the pot to be fully surrounded/covered by the insulating material. If the material does not touch the pot on all sides, or the pot is merely left on a heat source without wrapping, it is shehiyah, not hatmanah.
