Must one wash hands (netilat yadayim) before Minchah and Arvit, and is a blessing recited?

Synopsis

The Mechaber rules that one must wash hands before Minchah (and by extension Arvit) if water is available, even without known dirt, but does not recite a blessing. The Rema adds that even one who rises from Torah study must wash. If no prepared water is available, washing is not required.
