What is the normative Ashkenazi practice regarding the physical form of Nefilat Apayim — true prostration vs. merely tilting the head?

Synopsis

Earlier generations performed Nefilat Apayim as actual kidah (face to the floor, without spreading hands and feet). Current Ashkenazi practice is merely to tilt the head and cover the face. The face must be covered with a garment, not just with the hand.
