What constitutes 'niktam rosho' (top severed) that invalidates a lulav?

Dispute whether niktam rosho means the majority of upper leaves are severed (Mechaber, following Rosh) or the single central upper leaf is severed (Rema, following Maggid Mishneh and Ran). The Rema adds that if no other lulav is available, one may bless on it even with the central leaf severed.
