Must one concentrate (have kavanah) when answering Kaddish?

Authorities agree that concentration is required when answering Kaddish, as the Talmud teaches that one who answers Yehei Shemeih Rabba with full intent has a harsh decree torn up. Mere lip service without inner attention is insufficient.
